Tibialis posterior dysfunction

The tibialis posterior tendon runs behind the inner ankle and helps support the arch. If it becomes damaged or weak, the arch can gradually flatten and the foot may roll inwards.

What is tibialis posterior dysfunction?

The tibialis posterior tendon runs behind the inner ankle and helps support the arch. If it becomes damaged or weak, the arch can gradually flatten and the foot may roll inwards.

What causes it?

There may be more than one cause or contributing factor. These can include:

  • Wear and changes within the tendon over time
  • A sudden injury
  • Repeated strain
  • In some cases, no clear cause is found

What are the symptoms?

Everyone experiences symptoms differently, but you may notice:

  • Pain or swelling along the inside of the foot and ankle
  • A lowering of the arch
  • The heel rolling outwards or the foot rolling inwards
  • Difficulty walking or rising onto tiptoe
  • Increasing stiffness or arthritis as the problem progresses

How is it diagnosed?

Your consultant will examine the foot while you stand and move. X-rays show its shape and any arthritis, while an MRI scan can assess the tendon.

We’ll explain any tests clearly and talk you through what the results mean for you.

How is it treated?

Treatment depends on the cause, the severity of your symptoms and the activities you want to return to. We’ll help you understand your options before you decide what happens next.

  • Rest and a supportive boot while pain settles
  • An insole or brace to support the arch
  • Physiotherapy
  • Rigid custom footwear for a stiff or arthritic foot
